  At Ascension Social Care, we provide tailored support to young people aged 16+ who are preparing to leave care and transition into adulthood. Our supported accommodation services focus on building life skills, emotional resilience, and independence in a safe, nurturing environment.
  Each care leaver is encouraged to take ownership of their future through:

  • Personalised support plans developed collaboratively with the young person.
  • Help with managing finances, cooking, cleaning, and daily responsibilities.
  • Access to education, employment or training pathways.
  • Support in understanding their rights, including pathway planning and access to personal advisors.
  • Emotional and mental health support, including access to counselling or therapeutic services if required.
